Indications

Avatrombopag is a drug that stimulates platelet production and is used to treat the following conditions:

  • Idiopathic thrombocytopenia (ITP) : For the treatment of adults and children (≥ 1 year) with idiopathic thrombocytopenia, particularly when other treatment options (e.g., hormone therapy) have failed.
  • Thrombocytopenia associated with chronic liver disease : For the treatment of patients with chronic liver disease who need to increase their platelet count due to thrombocytopenia when planning surgery.

Mechanism of action

Avatrombopag is an oral small molecule platelet production stimulator (TPO receptor agonist) that binds to the thrombopoietin (TPO) receptor, activates the JAK2 signaling pathway, and promotes megakaryocytes in the bone marrow to produce more platelets, thereby increasing platelet counts. It can significantly improve platelet counts and help reduce bleeding symptoms caused by thrombocytopenia.


Dosage

  • Idiopathic thrombocytopenia (ITP) :

    • Recommended dose: 20 mg orally, once a day.
    • Duration of medication: The course of treatment is adjusted according to the condition. Usually the initial treatment lasts for 2-4 weeks. The doctor will adjust the treatment plan according to the platelet count.
  • Thrombocytopenia associated with chronic liver disease :

    • Recommended dose: 40 mg per oral dose, usually started 5-7 days before surgery.
    • Duration of medication: The treatment period depends on the patient's surgery time and changes in platelet count, usually not more than 5 days.
  • Directions :

    • Avatrombopag should be taken with food to reduce gastrointestinal discomfort.
    • Swallow the tablet whole; do not crush, chew, or split.

Precautions

  1. Side Effect Monitoring :

    • Common side effects : headache, fatigue, diarrhea, joint pain, nausea, etc.
    • Serious side effects : abnormal liver function, thrombosis (such as venous thrombosis, pulmonary embolism), allergic reactions, etc.
  2. Use in special populations :

    • Pregnant and lactating women : The safety of Avatrombopag in pregnant women is not clear, and its use should be avoided during pregnancy. Lactating women should consult a doctor before use.
    • Children : For children 1 year old and older, Avatrombopag can be used for idiopathic thrombocytopenia, but they should follow their doctor’s specific instructions.
    • Elderly patients have poor tolerance to drugs and should be particularly careful when using them. The dosage needs to be adjusted according to the specific condition.
  3. Liver function monitoring :

    • Liver function should be monitored regularly while using avatrombopag, especially in patients with a history of liver disease.

Drug interactions

  • CYP3A4 inhibitors : such as ketoconazole and erythromycin, may increase the blood concentration of Avatrombopag and should be used with caution.
  • CYP3A4 inducers : such as rifampicin and carbamazepine, may reduce the efficacy of Avatrombopag and should be used with caution.
  • Other drugs : Caution should be exercised when used in combination with other drugs that may affect platelet count or have anticoagulant effects, especially in patients at risk of thrombosis.

Contraindications

  • Patients with hypersensitivity to Avatrombopag or any of its ingredients are contraindicated.
  • It is forbidden for pregnant women to use.
  • It is contraindicated in patients with severe hepatic impairment (Child-Pugh score C).
  • Use with caution in patients with thrombotic disease.

Storage conditions

  • Storage temperature is 20°C to 25°C.
  • Avoid moisture, heat and direct sunlight, and keep the medicine out of the reach of children.

Important Tips

  • Patients should have regular blood tests, particularly liver function and platelet count monitoring, while taking Avatrombopag.
  • If a thrombotic event (such as deep vein thrombosis, pulmonary embolism, etc.) occurs, the medication should be stopped immediately and the doctor should be contacted.
