Pharmacological action and mechanism
Tralagliptin is a long-acting dipeptidyl peptidase-4 (DPP-4) inhibitor that inhibits the activity of the DPP-4 enzyme, delays the degradation of glucagon-like peptide-1 (GLP-1) and glucose-dependent insulin-releasing polypeptide (GIP), enhances insulin secretion, and inhibits glucagon secretion, thereby improving blood sugar control.
Indications
- For the treatment of type 2 diabetes mellitus (T2DM) in adults, as an adjunct to diet and exercise to improve blood sugar levels.
- It can be used alone or in combination with other glucose-lowering drugs (such as metformin and sulfonylureas).
Dosage

Recommended dose
- 100 mg orally once a week.

- Can be taken with or without food.
- It is recommended to take the medication at a fixed time, ensuring that the interval is consistent every week.

Special Populations
- Patients with impaired renal function : Patients with moderate or severe renal impairment require dose adjustment or use with caution.
- Elderly patients : No special adjustments are required, but renal function needs to be monitored.
Contraindications
- Patients with hypersensitivity to tralagliptin or any of its ingredients.
- People with type 1 diabetes or diabetic ketoacidosis.

Risk of hypoglycemia
- The risk of hypoglycemia may be increased when used in combination with insulin or sulfonylureas, and the dose should be adjusted.

- Patients with a history of acute pancreatitis should use this drug with caution. If persistent abdominal pain occurs, the drug should be discontinued and the pancreatic condition should be evaluated.

CYP450 enzymes
- Telagliptin is mainly excreted through the kidneys and is not metabolized by CYP450, so the risk of interaction with CYP450-related drugs is low.
Overdose
- Symptoms : May include severe hypoglycemia or gastrointestinal upset.
- Treatment : Symptomatic treatment, no specific antidote.
