The instructions for Dulutegravir, Emtricitabine, and Tenofovir Alafenamide (trade name: PHODETA ) contain the following content for reference. Please follow your doctor's instructions for specific use.
Drug Name
Generic name: dorunavir, emtricitabine, afanavir dipivoxil <br>Trade name: PHODETA
Indications
Symtuza is a combination antiviral medicine indicated for the following conditions:
- Treatment of human immunodeficiency virus (HIV) infection : As part of the treatment of HIV infection in adults and adolescents (≥12 years, weighing ≥40 kg), usually in combination with other anti-HIV medicines.
Symtuza is a fixed-dose combination medicine that contains:
- Dolutegravir : An Integrase Strand Transfer Inhibitor (INSTI) used to inhibit HIV viral replication.
- Emtricitabine : A nucleoside reverse transcriptase inhibitor (NRTI) used to stop the HIV virus from replicating.
- Tenofovir Alafenamide (TAF): Another nucleotide reverse transcriptase inhibitor (NRTI) that blocks HIV virus replication by inhibiting its reverse transcriptase function.
Mechanism of action
- Dorunavir : By inhibiting the action of HIV integrase, it prevents the integration of HIV virus DNA into the genome of host cells, thereby effectively inhibiting the replication of the virus.
- Emtricitabine : As a reverse transcriptase inhibitor, emtricitabine can bind to HIV RNA, preventing the virus from converting into DNA, thereby inhibiting viral replication.
- Alfanavir dipivoxil : As an NRTI, alfanavir dipivoxil reduces HIV replication by inhibiting the activity of reverse transcriptase and exerts an antiviral effect.
The combined use of three drugs can significantly reduce HIV viral load and improve immune function.
Dosage
-
Adults and adolescents (≥12 years, weighing ≥40 kg) :
- The recommended dose is one tablet, once a day , orally, preferably with food to increase drug absorption.
-
Directions :
- Swallow the tablet whole without chewing, crushing or splitting it.
- It can be taken with food to help improve drug absorption and reduce adverse reactions.
Precautions
-
Side Effect Monitoring :
- Common side effects : headache, diarrhea, nausea, fatigue, weight gain, abnormal liver function, etc.
- Serious side effects : liver problems (such as jaundice, liver failure), lactic acidosis, osteoporosis, kidney damage, etc.
-
Use in special populations :
- Pregnant women : Be especially careful when using during pregnancy, and it is best to use under the guidance of a doctor. The safety of dorunavir during pregnancy is not yet fully understood, and the risks and benefits must be assessed when using it.
- Breastfeeding women : Breastfeeding is not recommended because HIV drugs may be passed to the baby through breast milk.
- Children : Not recommended for children weighing less than 40 kg.
- Elderly : The dosage should be adjusted according to the physical condition of the elderly patient, especially liver and kidney function.
-
Liver and kidney function monitoring :
- When using this drug, liver and kidney function should be checked regularly, especially for patients with a history of liver or kidney disease. If abnormalities are found, the treatment plan should be adjusted in a timely manner.
-
Drug Interactions :
- Combination with other anti-HIV drugs : When used in combination with other anti-HIV drugs, drug interactions should be carefully checked to avoid side effects and reduced efficacy.
- Strong CYP3A4 inducers or inhibitors : Drugs such as carbamazepine and rifampicin may affect the metabolism of this drug and should be used with caution.
Contraindications
- It is contraindicated for patients who are allergic to dorunavir, emtricitabine, and afanavir dipivoxil .
- It is not recommended for patients with liver failure or severe renal impairment (e.g., patients with renal clearance less than 30 ml/min).
- Patients with severe lactic acidosis or abnormal fat distribution should avoid its use.
Storage conditions
- Store below 25°C , avoid high temperature and humidity.
- The medicine should be stored out of reach of children .
Important Tips
- During the use of this medicine, patients need to regularly check their liver function , kidney function and viral load to ensure the effectiveness and safety of the treatment.
- This drug does not cure HIV infection and cannot completely eliminate the virus in the body, so patients still need to use this type of antiviral drug for a long time to maintain viral control.
- Take the medicine strictly according to the doctor's instructions. Do not stop the medicine or adjust the dosage on your own to avoid viral rebound.
