Pharmacological action and mechanism
Crizotinib is a multi-target tyrosine kinase inhibitor that mainly acts on:
- Anaplastic lymphoma kinase (ALK)
- ROS1
- c-Met (HGF receptor)
By inhibiting these kinases, crizotinib blocks cell proliferation and tumor growth signals associated with these pathways.
Indications

ALK-positive non-small cell lung cancer (NSCLC)
- For the treatment of patients with advanced or metastatic NSCLC who are positive for ALK gene rearrangement.

ROS1-positive non-small cell lung cancer
- For the treatment of patients with advanced or metastatic NSCLC with positive ROS1 gene rearrangement.
Dosage

Recommended dose
- 250 mg, orally, twice a day.

Directions
- Can be taken with or without food, with water.

Dosage Considerations
- Depending on adverse reactions (such as abnormal liver function, gastrointestinal discomfort, etc.), the dose can be reduced to 200 mg twice a day, or further reduced to 250 mg once a day.
Contraindications
- Patients with hypersensitivity to crizotinib or any of its components.
Precautions

Liver function monitoring
- Crizotinib may cause liver damage. It is recommended to monitor liver function (ALT, AST, bilirubin) regularly before and during treatment.

Interstitial lung disease/pneumonia
- New or worsening respiratory symptoms (e.g., cough, shortness of breath) should be discontinued and the patient evaluated immediately.

QT interval prolongation
- It is recommended to monitor ECG and use with caution in patients at high risk of QT prolongation.

Visual impairment
- Some patients may experience visual symptoms (eg, blurred vision, double vision), which are usually mild and reversible.

Childbirth and breastfeeding
- Crizotinib may cause embryo-fetal toxicity. Female and male patients of childbearing age should take effective contraceptive measures. Breastfeeding women should avoid using it.

Drug interactions

CYP3A4 inhibitors/inducers
- Strong CYP3A4 inhibitors (such as ketoconazole) may increase the plasma concentration of crizotinib, and strong inducers (such as rifampin) may reduce its efficacy.

Other QT-prolonging drugs
- Caution is advised when using this drug in combination with drugs that may prolong the QT interval.
Overdose
- Symptoms : May include gastrointestinal discomfort, QT interval prolongation, etc.
- Treatment : There is currently no specific antidote, so the drug should be stopped immediately and symptomatic supportive treatment should be given.
