Indications

Tenofovir alafenamide is an antiviral medicine used primarily to treat the following conditions:

  1. Chronic hepatitis B virus infection :
    • For the treatment of adult patients with chronic hepatitis B, especially those who have received anti-hepatitis B virus treatment.
  2. HIV-1 infection :
    • As a component of combination antiretroviral therapy (cART) for the treatment of HIV-1 infection in adults.

Mechanism of action

Tenofovir alafenamide is a nucleoside (acid) reverse transcriptase inhibitor (NRTI) that blocks the replication of hepatitis B virus and HIV by inhibiting reverse transcriptase activity, thereby effectively reducing viral load and helping to control infection.


Dosage

  • Recommended dose :
    • For the treatment of chronic hepatitis B : 25 mg orally, once daily.
    • Treatment of HIV-1 infection : used in combination with other antiretroviral drugs, 25 mg orally once daily.
  • Directions :
    • May be taken with or without food. Tablets should be swallowed whole and should not be crushed, chewed or split.

Dosage adjustments :

  • For patients with renal insufficiency, the dose needs to be adjusted according to the renal filtration function.

Precautions

  1. Side Effect Monitoring :

    • Common side effects : headache, nausea, diarrhea, fatigue, insomnia, loss of appetite, etc.

Drug interactions

  • Nephrotoxic drugs : When used in combination with other nephrotoxic drugs (such as aminoglycoside antibiotics, antifungal drugs, etc.), attention should be paid to renal function monitoring.
  • CYP3A4 inducers : such as rifampicin and St. John's wort, may reduce the efficacy of tenofovir alafenamide and should be avoided in combination.
  • Combination with other NRTIs : Combination with other known nucleoside reverse transcriptase inhibitors should be avoided to reduce the risk of drug resistance.

Contraindications
